Federal contractors and subcontractors were previously required to use Form CC-503 to collect disability status from applicants and employees. However, OFCCP's final rule from August 21, 2026, ended the use of this form. Employers that wish to continue to voluntarily collect disability data can use the template created by DCI Consulting.
